- There are two simple steps to becoming a Quit & Win Participant.
Step 1: Online Registration Complete the
Online Registration Form
anytime between October 1 and October 29,
2007. Alternatively, registration may be
completed by fax (612-625-2925) or phone
(612-626-5520). After 5 p.m. October 29, you
must
complete this step in-person at the Quit & Win
On-Campus Enrollment Table on your campus on
October 30 or 31.
Step 2: On-Campus Enrollment After completing Online Registration, you will need to stop by the On-Campus Enrollment
Table on October 30 or 31 (click here for times and locations on your campus). At On-Campus Enrollment, you will be asked to do the following
(which takes less than 15 minutes):
- Fill out a brief survey about your current and past use of tobacco.
- Submit a urine sample. We use this to verify that you are a current smoker by testing for derivatives of nicotine.
- Receive quit aids like nicotine
patches or gum or sign up to receive support calls from a Quit
Coach.

| |
- Use of cessation aids is permissible and encouraged during the contest. During Step 2 of the enrollment, you will be offered several resources and cessation aids. You may also go to the Quitting Tips & Resources page on this website, or contact Maria Rangel at (612) 626-5520 for more information on what types of cessation aids and resources you can access.
- To be eligible for the final prize drawing, contest participants must:
a) Be smoke-free for the entire month of November (not even a puff).
b) Be willing to answer a brief post-contest online survey between December 1 and December 4, 2007 to declare their smoke-free status.
c) Be willing to submit a second urine specimen for lab verification of smoke-free status if chosen from a random drawing from all self-reported smoke-free participants.
- Final contest winners agree to have their first and last names in all Quit & Win promotional materials and related events.
- The final contest winners will be announced by December 17, 2007.
- The Quit & Win contest is not open to members of the Quit & Win contest planning team, those staff representing sponsors,
or their immediate family members.
- There is no exchange of prizes for cash value.
